How to Access Your Eye Care Records

1. Click the login link below or visit: https://secure.myeyecarerecords.com/

2. Click first time users - Register.

3. Your name must be entered exactly how it appears in our system along with your social security number. Your initial password is 1234. Our patient portal is provided by our EMR system and will prompt you to change it.  

4. You may also send your doctor a message via the patient web portal. Questions will be answered typically within 48 hours.
